Order Confirmation

1. This form provides required information for your Order and Invoice

2. You are electing to pre-purchase components that are in production

3. All payments must be made within 60 days of receiving your invoice

4. Components selected will be held no more than 90 days from the date this form was completed.

5. A $200 late fee will be added every 30 days beyond the initial 60 days of the agreed payment plan.
